- Residents
of Mendocino County.

Uninsured children ages 0 to 19th birthday regardless
of immigration status.
- A
family of 4 can make up to $5,000 a month and still
qualify.

What
you will need to enroll
Call Healthy Kids or one of the enrollment sites to
help you determine what you will need to enroll. Here
are examples of what you might need:
1.
Proof of Mendocino County Residency:
- pay
stub that includes your home address, or
-
utility bills, or
-
rental agreements, or
-
driver’s license or state identification card
2.
Proof of Income
- copy
of a recent pay stub, or
-
employer statement of gross monthly income, or
-
last year’s federal income tax return, or
- three
month profit and loss statement
-
for those without written proof of income, the enrollment
assistants can help you produce a statement of income.
3.
Other Information
